How to Contact LRPPH

ServiceLink provides information and referral services to seniors, adults with disabilities and their families. ServiceLink staff answer questions on issues related to health and wellness, legal, transportation, financial, housing, caregiving, home assistance and more.  In addition, ServiceLink provides: education and assistance with enrollment for the Medicare Part D prescription program, case management and financial eligibility services for individuals needing home and community based care/nursing home and other services.  In the past year volunteer and paid staff responded to 5,335 inquiries via phone, office and home visits. In addition, family caregiver advocates provided ongoing education, training, recreation and support services to over 265 family caregivers in Belknap County. This past year, ServiceLink staff  have provided educational services to the community on issues such as Medicare, identity theft, consumer debt, fraud/scams and caregiving as well as publishing a bi-weekly informational column in the Laconia citizen.

In addition to the work provided through the ServiceLink Resource Center, the Public Health Research has taken the lead in convening various providers such as Red Cross, fire chiefs, town managers, school nurses, police, home health, behavioral health, clergy and other citizens to establish plans in the case of a public health emergency such as an avian flu pandemic. Currently, over 60 people cross representing the community are involved in this planning process.

One of our goals this year is to construct a community website.  As you can see, this work has begun. This web site will provide current public health related information on issues such as flu, health and wellness and educational opportunities.  In addition, this site would portal to a searchable resource database.  Citizens in our region will be able to seek information about area services through the Internet.  Residents would always be welcomed and encouraged to call ServiceLink for more comprehensive information and assistance.

Continued financial support from the community will assist us in obtaining the resources we need to provide comprehensive services to a greater number of people in our community.
